"Appoint someone you trust to make decisions if you can’t."

A time may come when you may not be able to make decisions about your own health care. So today, while you’re still in fine fettle, think about the most likely person to act on your behalf. This might be a spouse, a child, or even an attorney. You’ll want to record your choice in a legal document called a health-care proxy. You can print a proxy off the Internet or get one from a lawyer. Just be sure you have the appropriate one for your state and that you sign it.



Health Care Proxies
What?
A legal document naming a person to make your medical decisions if you cannot. You specify in advance what steps you want taken if you are terminally ill, unconscious, or unable to speak.
Why?
To ensure your wishes are followed in event you cannot speak for yourself.
How?
Fill out a health-care proxy form and sign it. Keep a copy with your important papers; give a copy to your doctor and to the person you named ‘proxy.’
